Registration Information
We suggest that you contact Dr. Al before mailing your registration form to insure placement in the seminar since we only accept 10 participants in Florida and 6 in Massachusetts. DO NOT MAIL YOUR REGISTRATION FORM UNTIL YOU HAVE TALKED TO DR. AL. You can reach Dr. Al by telephone by calling 978.494.2546 or by sending an email to dr.al@comcast.net.
The total seminar fee for scheduled seminars is $525.00 USD and $625.00 USD for Seminars on Demand. Both fees include the first day’s lunch. A $275.00 registration fee should accompany the registration form and the balance paid on the first day of the seminar.

Early registration is recommended to insure admission. You will receive confirmation of your registration and hotel information promptly.
Cancellation
Requests for cancellation must be made in writing (including by email) and postmarked at least 15 days prior to the seminar. Because of the limited seminar enrollment, a $175 fee will be deducted from the deposit and the remainder will be refunded. GNEAH will issue a credit for any refundable amounts good toward any future seminars if the cancellation is postmarked or emailed within 15 days of the scheduled seminar. Credit not used within one year of the cancelled seminar will be forfeited. No refund or credit will be issued if you fail to meet the above requirements.
An income tax deduction may be allowed for education expenses to maintain or improve professional skills, subject to certain limitations and restrictions. You should consult your tax advisor for deductions in your tax status.
A certificate of attendance will be given to persons who attend the seminar.
These seminars have been approved as follows:
American Association of Nurse Anesthetists approves each seminar for 20 Continuous Education Credits.
